We have made our New Patient Forms available online. For your convenience, we recommend printing and completing these forms at home and bringing them to your first appointment. If you have any problem getting these files to print properly, please call our office at 317-776-8600 and we will gladly email, snail mail or fax them to you.

Dental Emergency Care Policy
Dental emergency care is available after hours for all our patients of record. Please call our main office number 317-776-8600 for assistance. If you are not a current patient, we will be happy to accommodate dental emergency cases during regular office hours. If it’s a true dental emergency and you need to see a dentist over the weekend or for a late evening appointment, there are several Emergency Dental Offices located in Indianapolis.
Insurance
Our insurance coordinator can handle all questions about claims and benefits during regular office hours. Our goal is to make our services as affordable as possible for our patients, which is why we offer flexible payment options. Be sure to ask about CareCredit, which can divide up any outstanding patient account balances into easy and affordable monthly payments. We participate with most major insurance plans: Aetna, Ameritas, Cigna, Delta Dental, Guardian, Blue Cross Blue Shield, Humana, MetLife, Principal, United Concordia and United Healthcare to name a few.
